The Norwood-Hamilton scale of male-pattern baldness

Typically, men experiencing male pattern baldness will remain with a permanent wreath of hair surrounding the sides and the back of their head, this is where hair is harvested from for transplantation. This hair is genetically programmed to continue to grow even in the worse cases of male pattern baldness. This hair is then surgically implanted into the balding area, to produce permanent results.

Follicular Unit Transplantation (FUT)

Follicular unit transplantation, or the transplantation of one follicular unit at a time, has become the standard of practice for hair transplants. It provides a much more natural appearance than the old plug grafts.

Natural Look, FUT

Over the past 10 years, due to significant technical and surgical advancements, the outcomes of Hair transplant surgery have dramatically improved. In the past with the “Plug” procedure, it was obvious that you had had a transplant as the hair was transplanted using large grafts, that produced a “dolls hair” effect. With today’s modern “FUT” ( Follicle Unit Transplant) procedures, that are performed as a minor outpatient treatment, you achieve a completely natural look and it is impossible to detect that you have had a hair transplant surgery. The permanent results are amazing and can transform the way you look and feel. FUT has proven itself over the decades and created millions of satisfied patients in the US.

Single hair grafts are utilized to re-create a natural appearing hairline whilst the larger units are positioned in other areas to produce density. Using a variety of graft types which combine the naturally occurring hair bundles, we are achieving density not seen previously. Once the grafts are prepared, Dr. Jane and her technicians will create tiny openings in the scalp into which the grafts will be later placed under local anaesthesia. Early results can be seen after 3-4 months and final results commonly seen after 12-15 months.

There are 2 types of FUT:

  • FUE ( Follicula Unit Extraction) with Neograft
  • Strip Grafting

The only difference between FUE and Strip grafts is how the follicular units are harvested. In FUE, the grafts are harvested individually from the scalp. In strip grafting, a strip of hair-bearing scalp is removed, and the follicular units are harvested from it. Each type of FUT has its advantages.

Strip Grafting

Once your scalp is numb after the local anesthesia, the strip surgery involves the surgeon removing a small area of the skin from the back of the scalp. The size depends on the size of the hair loss area to be treated. Because local anesthesia is used, there is typically little discomfort during the treatment.

The small incision, where the skin is taken from, is closed using either dissolvable sutures or sutures that can be taken out after 7 days. At the end of the procedure a small dressing is placed on the back of the hair where the hair has been taken from.

The tissue is then carefully dissected by a team of specialists, to extract the follicular unit grafts. The grafts are then visually inspected to select the healthy grafts to be implanted.
The surgeon then makes micro slits in the scalp and carefully places the grafts into the scalp.

 

There are several key elements necessary to achieve a satisfactory result. The most important of these key elements are the following:

  • Proper donor extraction and closing
  • Proper follicular unit dissection
  • Proper graft hydration and storage
  • Recipient site creation using proper angulation, direction and arrangement
  • Natural hairline design
  • Careful placement of grafts with the proper technique so as to avoid their damage
  • Proper post-surgery care

Advantages of Strip Grafting

Strip grafting has several advantages, too. It is less expensive and usually requires fewer sessions to get a satisfactory result than FUE. You don’t have to have your head shaved prior to the treatment as per FUE.

More follicular units are available to transplant with strip grafting. This is because the entire donor area is removed, and the scalp is sewn together. All of the follicles on the removed strip can be used for grafting. With FUE, however, many of the follicles must be left intact. Otherwise you would have a bald strip on the back of your head.

The follicular units harvested from strip grafts can be healthier and more robust than follicular units obtained by FUE. This is because the grafts can be dissected out of the scalp under the microscope, so the technician can actually see that she is getting the entire follicular unit. With FUE, each unit is removed directly from the scalp, so you can’t see exactly what you are getting. This can result of 2% to 4% potential loss of implanted hair with FUT compared to up to 10% with FUE.

FUE (Follicular Unit Extraction)

The only difference here is the method of extracting the donor hair. It certainly is not a more advanced technique nor does it have any advantage as far as the transplanted hair is concerned. In FUE, the individual follicular units are extracted using on average 0.8 – 1mm ‘drill bits’. Instead of one fine-line scar, there will be a large number of very small dot scars. It is a lot more time consuming and therefore more limited in the number of grafts possible to transplant in a session making it more expensive.

In our opinion, this method may be appropriate where a small number of hairs are required or where hair needs to be harvested from other body parts. It could be used to refine old style plug surgery. It is unfortunate that in some quarters FUE has been promoted as a ‘scar-less’ transplant.

Advantages of FUE

One big advantage of FUE is that there is no linear scar, no incision and no stitches. This makes the healing time shorter and virtually eliminates the chance of an infection. Elimination of the scar is especially important for people who like to wear their hair short and for people who tend to develop excessive scarring.

FUE is recommended for people with tight scalps. It is hard to close the incision after removing a strip graft on these people. It is also good for people who have scars from previous strip grafts, because they have less donor tissue remaining.

Dr. Jane offers follicular unit extraction (FUE), a method that uses robotic technology to independently extract hair follicles, as well as traditional strip or linear harvesting. Dr. Jane and her team select the most appropriate harvesting method for each individual patient, and no matter which procedure is selected, seek to minimize discomfort and maximize harvest yield, while protecting nerves and blood supply to promote better healing.

All harvested grafts are meticulously prepared for implantation and expertly implanted to achieve perfect angles, direction, pattern and graft-to-site fit.

Hair Transplant Surgery – The procedure Step by Step

Step 1: Donor Hair Extraction

The primary goals for the surgeon are to extract the requisite number of grafts, minimize damage to the donor area, and maintain a pleasant healing area using elite suturing technique. It is critical that your surgeon uses the techniques to minimize scarring in the donor area so that there is an aesthetic result and it allows for future surgery to improve density and surface area coverage. The angle of excision is critical which is vastly under appreciated by most hair surgeons. We close the area using the most proven techniques to produce minimal scarring.

Step 2: Graft Preparation

We use high magnification microscopes to dissect the tissue that was removed. It is mainly a quality control mechanism which can produce the greatest number of grafts with the fewest number of incisions, thus minimizing transection. The major damage to the extracted tissue takes place during the graft preparation stage and well trained technicians and microscopes aid this task. The prepared grafts are stored in physiologic solutions and maintained at cool temperatures to aid their survivability.

Step 3: Preparing the Scalp

The recipient sites are created using several instruments that vary in size and shape leaving the patient with a natural result but also creating density. Density can only be created with more hair per square centimeter. The ultimate angle and distribution of follicles can help to improve the appearance of density and fullness.

Step 4: Implantation

Grafts are implanted by highly trained technicians using a very gentle precision technique to minimize damage to the follicle. Our team has been instructed to follow the angle of the recipient sites which were created by the Doctor. As such, we use very fine jeweller’s forceps that are angulated to mimic the natural angle of hair follicle. Most clinics focus exclusively on number of grafts in a given area.
